Obtain A New Mexico Birth Certificate

Vital records are available to immediate relatives


State law in New Mexico restricts access to birth records to the immediate family, including parents, grandparents, siblings and spouses only. There are three ways to obtain a New Mexico birth certificate: by mail, in person or using an expedited online service. There is a standard fee for obtaining a birth certificate, and the expedited service has additional fees. In order to obtain a New Mexico birth certificate, you will need to make your application to the New Mexico Bureau of Vital Records & Health Statistics.


Instructions


1. Fill out an application form (see Resources) for a New Mexico birth certificate. This application form is available online or in person at the Santa Fe state office. If you do not have an application form and cannot print one out, you may write a letter containing the following: the person's name, city and date of birth, both parents' full names, your relationship to the named person, the purpose for which you will use the birth certificate, and your name, address and signature.


2. Mail the application and the current application fee to:

This address is also found on the application form and the New Mexico Department of Health website. Allow four weeks to process this request.


3. Order an expedited birth certificate, if you prefer, through VitalCheck (see Resources), New Mexico's online order processing for vital records including birth certificates. The VitalCheck application asks many of the same questions as the paper application. You will need to know the name of the person, their date and place of birth, and declare your relationship to the named person. Additional fees are required to use VitalCheck and must be paid by credit card.


4. Order a birth certificate in person, if this is the option you prefer, by going to Counter Service at the Santa Fe State Office or the Albuquerque Stanford Public Health Office. You will be asked to fill out a paper copy of the application for a birth certificate. If you are applying in person, you will need to bring a piece of government issued photo identification, such as a driver's license or passport, to prove your identity.
